What Special Circumstances Can Affect Form 5498-sa?

For my Form 5498-SA, some points are understood and affect my Form 5498-SA.

  • One of the most misunderstood things is that many applicants think that this is a Tax form, but it is only a form that keeps a record for information purposes. 
  • If an applicant has more than one HSA or has more than one contribution, then he or she gets multiple Form 5498-SA, and all these forms must be used for reporting. 
  • There is also a misunderstanding that affects the Form 5498-SA that employer contributions are reported, but this form involves both employee and employer contributions during a tax year. 
  • There is also a misconception that Form 5498-SA has to be filed by the end date of 31 December of a tax year. However, in reality, the contribution can be made till 15th April, which is the tax filing deadline of the year.

    Here are some of the potential issues that can affect Form 5498-SA: 

    • If you have any incorrect or wrong information in your Form 5498-SA, then you need to contact your custodian and issue a correct form if it is necessary. 
    • In many cases, you can get various forms for one HSA. This is only possible if you are transferring between custodians in a financial year. 
    • HSA custodians need to issue Form 5498-SA by the end of May 31 in the current tax year. If you failed to do so, then you can contact your custodian about its status. 
    • If you have filled wrong details in your 5498-SA form, or your personal information does not match. In that condition, you need to report your HSA custodian.

      If the contributions are not filed properly by the employer or the employee to the HSA during the tax year, then it can affect Form 5498-SA.

      Providing incorrect information in the Form 5498-SA can affect you, and you need to again fill out a new Form 5498-SA with all the correct information and before the due date, 31 May.
